Maktab Transfer Form

This form must be completed by the previous maktab and submitted to the office when making an application to a new maktab. For this form to work efficiently, maktabs of the area must all agree with each other on this form being a requirement ensuring parents who are in arrears or children who may have some needs are looked after accordingly.
